This intervention is planned by evaluating the number and location of metastatic lesions and the patient’s general health condition. The aim is to control the spread of the disease and prolong life expectancy.

The choice of surgery in patients with lung metastasis depends on the primary origin of the tumor, the number of metastases, and the patient’s performance status. Suitability for surgery is determined by factors such as response to systemic therapy and whether the metastases are unilateral or limited.

Postoperative follow-up and the possibility of recurrence are monitored through regular radiological imaging and clinical evaluations. Since metastasis may develop again in some patients after lung metastasis surgery, long-term monitoring is of great importance.

Alternative treatment methods and combined approaches may be supported with radiofrequency ablation, chemotherapy, and immunotherapies in patients who are not suitable for surgery. In some cases, surgery is combined with systemic treatments and a patient-specific multidisciplinary plan is created.

Things You Need to Know

Information

Definition

Lung metastasis surgery is a surgical procedure performed when cancer that begins in another part of the body (primary) spreads to the lungs (metastasis). class="ez-toc-section" id="What_Is_Lung_Metastasis_Surgery">What Is Lung Metastasis Surgery?

Lung Metastasis Surgery is the surgical removal of metastatic foci that occur when cancer developing in another organ of the body spreads to the lungs. It is applied in suitable patients when the primary tumor is under control and the number of metastases is limited. The aim is to control the disease and prolong survival. The treatment plan is determined through multidisciplinary evaluation and is combined with systemic therapies when necessary.

How does the Multidisciplinary Council process work when making a treatment decision?

When metastasis is detected in the lung, this technically means that the disease is stage 4, that is, advanced stage. In such a situation, the treatment decision is too serious to be left to the initiative of a single doctor or a single surgeon and requires comprehensive thinking. For us, the “gold standard” is the structure we call the Multidisciplinary Tumor Council. In this council, we review the patient’s file together and create a common understanding by evaluating the case from the perspective of each specialty.

The specialists who must be present in this council are as follows:

Thoracic Surgery Specialist

Medical Oncologist

Radiation Oncologist

Thoracic Radiologist

Pathologist

Pulmonology Specialist

Nuclear Medicine Specialist

The presence of this team is of vital importance because not every lesion may be metastasis. Sometimes a completely new tumor originating from the lung tissue itself may have developed independently from the patient’s previous cancer. Making this distinction completely changes the treatment. In addition, the timing of surgery is also critical. For example, in a bowel cancer metastasis, should drug therapy (chemotherapy) be given first, or should surgery be performed first? The answer to this question is decided at this table according to the biology of the tumor and the general condition of the patient. As a surgeon, my duty is not only to technically remove that mass; it is also to ensure that this surgery is compatible with the other treatments the patient will receive and to manage the recovery process so that it does not interrupt those treatments.

Which patients are considered suitable candidates for lung metastasis surgery?

Unfortunately, we cannot operate on every patient who has lung metastasis. In order for surgery to benefit the patient without causing harm, it must meet some internationally accepted strict criteria, in other words a kind of checklist. The logic of “every patient with a nodule can have surgery” is incorrect. We wait for certain conditions to be met to ensure that we will benefit the patient.

The main criteria we look for in suitability for surgery are as follows:

The primary tumor being under control

The metastasis being limited only to the lungs

A high probability of complete resection (R0)

The patient having sufficient respiratory capacity

The cardiac reserve being sufficient to tolerate surgery

First of all, the main organ that is the source of the disease (for example the intestine or kidney) must have been treated and there must be no active disease remaining there. If the source is not controlled, cleaning the result has no meaning. Secondly, metastases must be limited to the lungs or, if present elsewhere (for example in the liver), they must also be removable. If the disease has spread throughout the body, systemic drug therapies come to the forefront instead of surgery, which is a local solution. In addition, the patient must have the capacity to continue normal life without shortness of breath with the remaining lung tissue after surgery. When these criteria are met, surgery may become possible even if the number of metastases is high.

What are the preoperative risk analysis and prognostic factors?

Before taking a patient into surgery, we do not only look at the current snapshot; we try to foresee the entire film. Some clues that we call “prognostic factors” give us signals about how the disease will behave in the future and how much benefit will be obtained from surgery. Analyzing these factors allows us to avoid unnecessary and exhausting surgeries or, on the contrary, to apply a more aggressive treatment.

Some important factors we evaluate in risk analysis are as follows:

High CEA levels

Short disease-free survival period

Mediastinal lymph node involvement

Presence of multiple nodules in both lungs

Histological subtype of the tumor

For example, very high CEA levels in the blood in colorectal cancer metastases may indicate that the tumor is aggressive. Similarly, the development of metastasis shortly after the first cancer surgery shows that the tumor is growing rapidly and makes us more cautious. However, the presence of these risk factors definitely does not mean that “surgery cannot be performed.” It only indicates that postoperative follow-up should be stricter and perhaps that the dose or duration of chemotherapy given before or after surgery should be rearranged. Although involvement in both lungs is a negative factor, if all lesions can be removed, surgery may still remain the strongest option on the table.

You can contact us via WhatsApp.
To get information about treatments, simply click the button next to it. As a surgeon, I must know millimetrically which part of the lung I will enter and which vessel I will work close to. In this mapping process, the two most powerful tools that technology offers us are Computed Tomography (CT) and PET-CT.

CT, that is tomography, provides us with anatomical detail. The location of the nodule, its size, and its relationship with surrounding structures are seen most clearly on tomography. However, tomography cannot tell us whether that nodule is “alive.” This is where PET-CT comes into play. PET shows metabolic activity by visualizing the sugar consumption of tumor cells.

The purposes of using imaging methods are as follows:

Determining the anatomical location

Detecting metabolic activity

Screening distant organs

Lymph node evaluation

Checking for brain metastasis

The greatest advantage of PET-CT is that it can show with high accuracy whether cancer cells are hidden inside a lymph node that appears normal in size. It also scans whether there is hidden metastasis elsewhere in the body, for example in the bone. However, PET may sometimes be misleading in nodules smaller than 1 centimeter. Therefore, we surgeons do not rely on a single image; we evaluate both images together and, if necessary, support them with brain MRI to minimize the risk of surprises.

How does lymph node status affect the surgical strategy?

In metastasis surgery, one of the most overlooked issues but also one of the biggest causes of failure is the status of the mediastinal lymph nodes located in the middle of the chest cavity. The basic logic of metastasis surgery is the assumption that the disease is limited only to the lung tissue. If cancer has passed the stations in the lung and spread to the lymph nodes in the mediastinum (a condition we call N2 disease), this indicates that the disease has become more systemic.

In such a situation, the benefit provided by surgery may decrease. Therefore, we must examine suspicious lymph nodes before surgery. For this purpose, we use methods called EBUS (Endobronchial Ultrasound), which allow us to visualize lymph nodes with ultrasound during bronchoscopy and take samples with a needle. Sometimes we also take samples from these nodes through a small procedure called mediastinoscopy.

The purposes of lymph node evaluation are as follows:

Preventing unnecessary surgery

Confirming the stage of the disease

Determining the need for chemotherapy

Defining surgical margins

If involvement in mediastinal lymph nodes is detected, we generally prefer to first control the disease with chemotherapy and postpone surgery to a later stage. Lymph node staging is our most important safeguard in order not to exhaust the patient unnecessarily and to apply the right treatment at the right time.

Is the closed method (VATS) or open surgery (Thoracotomy) preferred?

The question our patients ask most frequently in the clinic is undoubtedly, “Will my surgery be closed or open?” In lung metastasis surgery, both methods have their own advantages and disadvantages, and this decision is made entirely individually for each patient. Today, the method we call VATS, Video-Assisted Thoracoscopic Surgery, that is the closed method, is quite popular because it is minimally invasive.

Some advantages provided by the VATS method are as follows:

Less postoperative pain

Shorter hospital stay

Better cosmetic results

Faster return to daily life

Less tissue damage

In this camera-assisted procedure, we perform the operation by entering through small openings without separating the ribs. However, there is a very special situation in metastasis surgery: the “sense of touch.” On tomography, sometimes millimetric nodules as small as a grain of rice may not be visible. In open surgery (thoracotomy), the surgeon can insert their hand into the chest cavity and palpate the entire lung with their fingers, finding and removing these small lesions that cannot be seen on imaging. In closed surgery, we do not have this opportunity; we can only remove what we see on the screen. Therefore, if the patient has multiple nodules, if the nodules are located very deep in the lung, or if hard tumors such as sarcomas that can be palpated are involved, we may prefer open surgery to ensure complete oncological clearance. Here, the goal of achieving a cure takes priority over aesthetic concerns. In single and superficial nodules, however, VATS is unquestionably our first choice.

You can contact us via WhatsApp.
To get information about treatments, simply click the button next to it. However, in metastasis surgery, our philosophy is the opposite: “Remove the least possible tissue, preserve the most function.” Because in patients with metastasis, the possibility of recurrence always exists and the patient may need to undergo surgery again in the future. Therefore, the patient’s respiratory capacity should not be excessively consumed in the first operation.

The methods we use in tissue-preserving surgery are as follows:

Wedge resection

Segmentectomy

Laser metastasectomy

Use of precise cautery

The most commonly applied “Wedge resection” is the removal of the tumor in a triangular shape (like a slice of cake) together with some healthy tissue around it. Scientific studies have shown that in metastasis surgery there is no survival difference between removing an entire lobe and removing the nodule with a wide margin. For this reason, we resort to lobectomy only in very necessary cases.

In addition, “Laser” technology provides great convenience in this field. Especially in patients with multiple nodules, we have the chance to remove the tumor almost by “carving” it out with a laser beam without burning or cutting the normal lung tissue. The laser provides bleeding control while preserving healthy tissue to the maximum extent. Automatic stapling devices called staplers also allow us to close the tissue without causing air leakage. All these technologies aim to ensure that the patient leaves the operating table with minimal loss.

What should be the treatment sequence in colorectal cancer metastases?

Colorectal cancers are the group that most frequently metastasizes to the lungs. Metastasis is seen in approximately 20% of patients at the time of diagnosis. In this situation, treatment planning is almost like a chess game. If both the tumor in the intestine and the metastasis in the lung are present simultaneously (synchronous), the order of treatment becomes vitally important.

The treatment strategies we apply in these patients are as follows:

First intestinal surgery, then chemotherapy, and finally lung surgery

Simultaneous (same session) intestinal and lung surgery

First chemotherapy, then sequential surgeries after tumors shrink

First lung surgery, then intestinal surgery

The classic and most commonly applied approach is first removing the intestinal tumor that poses the main threat to the patient, then administering protective chemotherapy, and finally clearing the nodule in the lung. However, if the patient is young and fit, we may also consider performing both surgeries simultaneously. This relieves the patient of the entire burden in a single session, but the recovery process may be slightly more difficult. In the “reverse” approach, tumors are first suppressed with chemotherapy and surgery is performed after a response is obtained. Our greatest fear here is the progression of the disease during the waiting period between two-stage surgeries. Therefore, when making the decision, we weigh factors such as the risk of intestinal obstruction and the size of the lung nodule.

If there are metastases in both the liver and lungs, which surgery should be prioritized?

Another scenario we frequently see in colorectal cancers is the spread of the disease to both the liver and the lungs. In the past, these patients were considered to have lost their chance for surgery, but today surgery is definitely an option if metastases in both organs can be removed. So which should be done first?

The general approaches on this subject are as follows:

Performing the technically more difficult one (liver) first

Performing the one with faster recovery (lung) first

Simultaneous surgery in suitable cases

The generally accepted view is to perform the technically more difficult and risky liver surgery first. Because recovery after liver surgery may be more complicated and priority is given to the organ that carries vital risk for the body. Another school of thought argues that lung surgery should be performed first because it is better tolerated and has a much shorter recovery period. Sometimes liver and lung surgery teams may also perform simultaneous clearance in the same session. This decision depends entirely on the experience of the surgical teams and the patient’s resilience.

What are the success rates of surgery in different tumor types (Sarcoma, RCC)?

The success of surgery is not only closely related to the surgeon’s skill but also to the “identity” of the tumor, that is, its histological type. Each type of cancer behaves differently in the lungs.

The responses of different tumor types to surgery are as follows:

Carcinomas

Sarcomas

Renal Cell Carcinoma

Carcinomas, especially those of colorectal origin, constitute the largest group undergoing metastasectomy and are the group with the highest success rates. When complete clearance is achieved, 5-year survival rates are quite high. Sarcomas, that is soft tissue tumors, are more aggressive and may be resistant to chemotherapy. Therefore, in sarcomas, surgery is the most powerful weapon available for the patient. If the nodules in the lung can be completely removed, sarcoma patients also gain a significant survival advantage.

Renal Cell Carcinoma (RCC), that is kidney cancer, also frequently metastasizes to the lungs. This tumor type is highly resistant to chemotherapy and radiotherapy. Therefore, lung metastasis surgery is almost the only effective treatment method in kidney cancer patients. While survival is very low without treatment, these durations increase dramatically with surgery. In RCC metastases, “complete clearance” and the “disease-free interval” are the most important success criteria.

What are the survival expectations and long-term outcomes after surgery?

The oncological role of lung metastasis surgery has been proven through thousands of operations and scientific studies conducted over the years. In general, in selected patient groups, that is, cases where the tumor is completely removed and the patient’s general condition is good, 5-year survival rates vary between 30% and 70%. These rates are remarkable numbers for a metastatic, that is stage 4, cancer patient.

Considering that these rates are very low in patients who do not undergo surgery, the value of metastasectomy becomes clearer.
