The brachial plexus is most commonly affected, and patients may develop pain, numbness, and weakness in the arm, shoulder, and neck region.

The causes of Thoracic Outlet Syndrome may develop due to congenital rib anomalies, trauma, repetitive arm movements, and postural disorders. The syndrome is divided into three clinical types as neurogenic, venous, and arterial, and each shows different symptoms.

The diagnostic process and differential diagnoses are supported by physical examination tests, electromyography (EMG), color Doppler ultrasound, and imaging methods such as CT angiography. A multidisciplinary approach is required, especially in which neurological and vascular symptoms are evaluated together.

Treatment options and rehabilitation approaches bring surgical intervention to the agenda if a response cannot be obtained from conservative treatment. Physical therapy, posture training, pain management, and muscle relaxation techniques are preferred in the first step. In surgical treatment, the anatomical structure causing the compression is removed.

Things You Need to Know

Information

Definition

Thoracic Outlet Syndrome (TOS) is a syndrome that occurs as a result of compression of the vessels (subclavian artery/vein) or nerves (brachial plexus) in the thoracic outlet region located between the neck and chest. class="ez-toc-section" id="What_Is_Thoracic_Outlet_Syndrome">What Is Thoracic Outlet Syndrome?

Thoracic Outlet Syndrome is a clinical condition that occurs as a result of compression of the nerves, arteries, or veins in the narrow space between the collarbone and the first rib. It most commonly presents with symptoms such as pain, numbness, weakness, and swelling in the arm and hand. Causes include anatomical variations, trauma, and repetitive arm movements. Treatment may include physical therapy, medications, and surgical intervention in advanced cases.

What Are the Symptoms of Thoracic Outlet Syndrome (TOS) and How Does the Body Signal It?

The symptoms of this syndrome vary greatly depending on which structure is being compressed. There are three main structures passing through the tunnel: nerves (brachial plexus), artery, and vein. In the vast majority of our patients, almost ninety-five percent, the compressed structure is the nerves. We call this “Neurogenic TOS.” When the nerves are under pressure, the body’s response is based on pain and sensory loss. Patients generally complain of pain starting from the base of the neck, extending to the shoulder, then down the inner surface of the arm to the ring and little fingers of the hand. Sometimes this pain is so exhausting that patients do not even know where to place their arm.

The symptoms caused by nerve compression are as follows:

Neck pain

Shoulder pain

Hand numbness

Tingling

Loss of strength

Tenderness

Headache

Back pain

If the compressed structure is the vein, the picture is somewhat different and visually more obvious. This is called “Venous TOS.” When the vein is compressed, the deoxygenated blood in the arm cannot return to the heart and begins to pool in the arm. This condition can usually occur suddenly in young and active individuals, with a clot forming after intense arm activity or sports. Sudden swelling, bruising, and a feeling of fullness in the arm are the most typical findings. This is a condition requiring more urgent intervention compared to nerve compression because it carries a risk of clotting.

The complaints seen in the venous form are as follows:

Arm swelling

Bruising

Feeling of fullness

Fatigue

Prominent veins

Pain

The rarest but most dangerous type, which can even lead to limb loss if neglected, is the “Arterial TOS” type, in which the artery is compressed. Here, the flow of clean blood going to the arm is obstructed. The arm becomes undernourished. The patient gets tired quickly when using the arm and feels cramp-like pain. The hand and fingers are distinctly colder and paler than the other arm. In advanced cases, small non-healing wounds may be seen at the fingertips. The presence of this condition is a red line requiring a rapid surgical decision.

The signs of the arterial form are as follows:

Coldness

Paleness

Loss of pulse

Getting tired quickly

Cramps

Finger wounds

Which Causes Play a Role in the Development of Thoracic Outlet Syndrome?

The most common question we hear when talking with our patients is, “Why did this happen to me?” At the root of this syndrome lie two main categories. The first is the anatomical differences a person is born with. The second is conditions acquired later through lifestyle and trauma. The leading congenital causes include what we call a “Cervical Rib.” Normally, our neck vertebrae do not have ribs; ribs begin from the thoracic vertebrae. However, in a certain portion of the population, there may be an extra rib or rib remnant on the seventh cervical vertebra. This extra bony structure narrows that already tight tunnel even more and directly puts pressure on the nerves and vessels. Not everyone with a cervical rib develops the disease, but we often encounter this anomaly in those diagnosed with TOS.

In addition, modern living conditions can also trigger this syndrome. Particularly, posture disorders seen in desk workers are a major risk factor. Constantly drooping shoulders, the head extending toward the computer screen, push the collarbone downward toward the first rib. This chronic postural disorder leads over time to shortening and stiffening of the muscles in that area and narrowing of the tunnel. In addition, neck traumas (whiplash injuries) caused by traffic accidents or clavicle fractures can also cause adhesions in the region during healing and pave the way for the development of TOS even years later.

The factors predisposing to the development of TOS are as follows:

Cervical rib

Posture disorder

Neck traumas

Repetitive movements

Heavy lifting

Muscle imbalances

Bone anomalies

Fibromuscular bands

Why Is It Difficult to Diagnose Thoracic Outlet Syndrome (TOS)?

This syndrome is known in the medical world as the “Great Imitator.” This is because its symptoms resemble many common diseases such as cervical disc herniation, shoulder impingement, nerve entrapment at the elbow (cubital tunnel), or nerve entrapment at the wrist (carpal tunnel). Sometimes it can even be confused with fibromyalgia. Therefore, our patients often have to visit many different specialties such as Orthopedics, Neurosurgery, and Physical Therapy before reaching the correct diagnosis.

We do not have a single magic test to diagnose it. In other words, it may not always be possible to say “Yes, this is definitely TOS” with just a blood test or just an X-ray. The diagnostic process begins with listening very carefully to the patient’s story and a detailed physical examination. During the examination, we place the patient’s arm and head in certain positions to check whether the complaints are triggered or whether the pulse disappears. These provocative tests give us important clues.

Then imaging methods come into play. X-ray and Computed Tomography (CT) are used particularly to see the extra rib in the neck region (cervical rib) or abnormalities in the first rib. 3-dimensional CT images are extremely valuable in terms of mapping the bone structure. Magnetic Resonance (MR) is necessary to evaluate soft tissues, muscles, and nerve roots, as well as to rule out other possibilities such as a cervical hernia. The EMG test, which includes nerve conduction studies, helps us understand at which level the nerve is compressed; however, it should not be forgotten that EMG can sometimes also be normal in TOS patients. If vascular involvement is suspected, examinations such as Doppler ultrasound and angiography are also an integral part of the process.

The methods used in the diagnostic process are as follows:

Physical examination

Provocative tests

Plain radiography

Computed tomography

Magnetic resonance

Electromyography

Doppler ultrasound

Angiography

You can contact us via WhatsApp.
To get information about treatments, simply click the button next to it. Especially the great majority of patients diagnosed with neurogenic TOS, that is, those with nerve compression, can regain their health with a correctly planned conservative treatment program without the need for surgery. Our basic philosophy in treatment is that if permanent damage has not occurred in the nerve and the patient’s quality of life is tolerable, we first allow the body the opportunity to repair itself.

The most important pillar of this process is physical therapy. However, what is meant here is not a standard physical therapy but a personalized program specific to TOS. The aim is to widen the tunnel by correcting forward-dropped shoulders, stretching shortened neck and chest muscles, and strengthening the back muscles. Correcting posture lifts the collarbone upward and allows the nerves passing underneath it to breathe. In addition, with special exercises we call nerve mobilization, the goal is for the nerves to glide comfortably among the tissues.

In addition to physical therapy, medical treatment also supports the process. Painkillers, muscle relaxants, and neuropathic pain medications increase the patient’s comfort. The changes the patient makes in daily life also directly affect the success of the treatment. It is necessary to provide an ergonomic work environment, regulate phone and computer use, and avoid activities that keep the arm above head level for long periods. Generally, a disciplined treatment period of 3 months is foreseen. If the patient improves during this process, surgery is no longer on the agenda.

The components of conservative treatment are as follows:

Physical therapy

Posture exercises

Muscle strengthening

Stretching movements

Medication treatment

Ergonomic adjustments

Lifestyle changes

Nerve mobilization

In Which Situations Is Thoracic Outlet Syndrome Surgery Necessary?

Although we always prioritize non-surgical methods, in some cases surgical intervention becomes not a preference but a necessity. From the perspective of thoracic surgery, there are certain criteria that lead us to decide on surgery. The most important criterion is the failure of conservative treatment. If the patient does not experience a reduction in pain despite receiving regular physical therapy and medication treatment for at least 3 months, continues to have interrupted sleep, and has difficulty performing daily tasks, the surgical option is brought to the table.

In addition, the presence of progressive neurological damage eliminates our luxury of waiting. If serious loss of strength has begun in the hand, if wasting (atrophy) is observed in the hand muscles, this indicates that the nerve is heading toward irreversible damage. In such a picture, surgery should be planned without waiting for the 3-month period. The aim is to relieve the nerve from pressure before permanent damage occurs.

Vascular problems, on the other hand, are a completely different category and generally require urgent or priority surgery. If venous thrombosis is present, clot-dissolving treatment is applied first, and then the bony structure narrowing the tunnel is surgically removed to prevent the clot from forming again. In arterial compressions, the situation is more serious; if there is an aneurysm (ballooning) or occlusion, immediate intervention is required to prevent the risk of limb loss.

The conditions requiring surgery are as follows:

Resistance to treatment

Loss of strength

Muscle wasting

Unbearable pain

Venous thrombosis

Arterial aneurysm

Arterial occlusion

Findings of ischemia

You can contact us via WhatsApp.
To get information about treatments, simply click the button next to it. Think of it as a tunnel whose ceiling has collapsed and is crushing the cables passing underneath. What we do is free the cables by lifting this ceiling. To achieve this, the first rib causing the compression, if present the cervical rib, and the hardened scalene muscles surrounding the tunnel are generally removed. There are three main approaches used in surgery, and each has its own advantages:

The first method is the “Transaxillary” approach. In this method, entry is made through an incision in the armpit. It is cosmetically advantageous because the scar is hidden, and the first rib can be removed from here. However, the field of view is relatively more limited, and it may be difficult to reach the cervical rib in the neck or perform complex vascular repairs by this route. The second method is the “Supraclavicular” approach. Entry is made from the neck region through an incision just above the collarbone. This method allows control of the vessels and nerves from above and is preferred especially in cases requiring vascular repair or with major bone anomalies.

The third method, which has come to the forefront in recent years, is the “Posterior” or back approach (PURED technique). Entry is made through a small incision from the back, beside the shoulder blade. This method provides the surgeon with a bird’s-eye view. It allows direct access to the problematic bone without pulling or retracting the nerves and vessels. It enables the complete safe removal of the first rib from back to front. Complete removal of the rib is very important because leaving a small bone fragment inside may cause the disease to recur in the future. One of the greatest advantages of the posterior approach is that it enables the application of “fat grafting” (lipofilling).

The surgical techniques are as follows:

Transaxillary approach

Supraclavicular approach

Posterior approach

PURED technique

First rib resection

Scalenectomy

Cervical rib excision

Neurolysis

How Is the Risk of Recurrence Managed and What Is the Advantage of the PURED Method?

It is known that in some patients after TOS surgery, approximately 15-20 percent, the complaints can return. This is difficult for both the patient and the physician. The main cause of recurrence is that the “scar tissue” (fibrosis) produced by the body as healing tissue in the surgical area develops excessively and compresses the nerves again. In addition, failure to remove the rib completely in the first surgery and leaving a stump is also a cause of recurrence.

This is where the PURED technique and the accompanying fat transfer come into play. In this technique, after the surgical field is cleaned and the bone is removed, the patient’s own fat tissue taken from the abdominal area is placed around the released nerves like a cushion. This living fat tissue surrounds and protects the nerves and, most importantly, prevents hard scar tissue from adhering to the nerve. Fat tissue provides the most natural and slippery environment for the nerves. This strategy is a very promising development, especially for patients who fear recurrence or who have previously had surgery and developed recurrence.

The causes of recurrence and precautions are as follows:

Scar tissue

Insufficient surgery

Bone stump

Fat grafting

Total resection

Minimally invasive surgery

Early physical therapy

What Is the Recovery Process Like After Thoracic Outlet Syndrome Surgery?

The hospital stay after surgery is generally around 3 days on average. Recovery dynamics vary according to the type of syndrome. Patients with vascular compression feel the relief, color change, and warming in their arms as soon as they come out of surgery. However, in patients with nerve compression (Neurogenic TOS), the process requires a bit more patience. Nerve tissue is the slowest healing tissue in the body. Although the pressure is relieved by the surgery, the nerve takes time to repair itself internally. Pain generally decreases rapidly, but it may take between 6 months and 1 year for the feeling of numbness and tingling to completely disappear.

It is very important that the patient does not remain passive during this period. Physical therapy started early is the most critical factor reinforcing the success of the surgery. To preserve shoulder range of motion and prevent the development of “frozen shoulder,” controlled exercises are started with the surgeon’s approval. The recovery process is not just about the wound closing, but also about the nerves adapting to their new and comfortable environment and the muscles strengthening. Being patient and following the doctor’s recommendations to the letter is the key to full recovery. Remember, TOS is not your fate; with correct diagnosis and personalized modern treatment approaches, it is possible to return to a pain-free life.

The steps of the recovery process are as follows:

Hospitalization

Wound care

Pain control

Early mobilization

Physical therapy

Nerve healing

Strengthening exercises

Regular follow-up

Frequently Asked Questions

Why does thoracic outlet syndrome occur and in whom is it more common?

Thoracic outlet syndrome occurs when the nerves and vessels between the neck and the arm are compressed in a narrow space.
