This condition may appear as mild streaking or as heavy and clotted bleeding. Hemoptysis is a clinical finding of underlying respiratory system diseases and should be evaluated carefully.

Causes of bloody sputum include infectious diseases such as acute and chronic bronchitis, pneumonia, bronchiectasis, and pulmonary tuberculosis. In addition, serious pathologies such as lung cancer, pulmonary embolism, and traumatic lung injury may also lead to hemoptysis. Smoking is an important risk factor.

The symptoms of hemoptysis and the diagnostic process are planned according to the amount and duration of bleeding and accompanying symptoms. Physical examination, chest X-ray, and computed tomography are the basic imaging methods. When necessary, bronchoscopy is performed to identify the bleeding site and conduct further examinations.

The treatment of bloody sputum and its risk factors are determined according to the underlying cause. Antibiotic treatment is administered in cases caused by infection, while surgical or oncological treatments are preferred for tumoral formations. It Distinguished?

Not every instance of blood coming from the mouth is the lung-origin bleeding we are referring to. First of all, we need to clarify this distinction. Sometimes a problem in the gums, drainage of a severe nosebleed into the throat, or even simple irritation in the throat may cause blood to appear in the saliva. Even stomach bleeding may present as blood coming from the mouth and is often confused with pulmonary bleeding. However, blood coming from the lungs, meaning the airways below the vocal cords, has its own distinctive characteristics, and for us physicians, these details form the first step of diagnosis.

The characteristics of true bloody sputum are generally as follows:

Bright red color

Foamy structure

Alkaline pH value

Presence of clots

In contrast, bleeding originating from the stomach (hematemesis) is generally darker in color, resembles coffee grounds, and may contain food particles. A patient experiencing pulmonary bleeding usually describes a burning, tickling, bubbling, or warm sensation in the chest before the blood appears. The blood is then expelled with the cough that follows this sensation. If the bleeding originates from the stomach, nausea and the urge to vomit are generally more prominent beforehand. Being able to make this distinction is the most important clue that will save us time when you arrive at the hospital.

What Causes Bloody Sputum and Which Diseases Lead to It?

Our lungs are among the organs with the most complex and richest vascular networks in the body. Any damage, inflammation, or rupture occurring in this delicate structure may turn into blood expelled through coughing. Our clinical observations in Türkiye reveal that the causes of bloody sputum show certain differences compared with Western countries. In our country, infectious diseases and the permanent damage they leave behind are still among the most common causes we encounter.

Severe infections or diseases such as tuberculosis experienced especially during childhood may lead to structural deterioration in the lungs later in life, creating a predisposition to bleeding. In addition, the risk factors introduced by the modern era and smoking also have an important place in the equation.

The causes of bloody sputum that we frequently encounter are as follows:

Bronchiectasis

Tuberculosis

Lung cancer

Pneumonia

Lung abscess

Pulmonary embolism

Fungal infections

Trauma

Heart valve diseases

Foreign body aspiration

Bronchiectasis is a condition in which the airways become permanently widened and structurally damaged. Around these enlarged bronchi, the body forms new but highly fragile blood vessels as a repair mechanism. Since these vessels cannot withstand high pressure, they may rupture easily even with a severe cough and cause persistent bleeding. Tuberculosis, on the other hand, keeps the risk of bleeding constantly present by causing tissue necrosis during the active period and by leaving cavities and scar tissue during the healing period.

Lung cancer is a condition that must be ruled out, especially in individuals over the age of 40 with a long history of smoking. Tumors create irregular and weak-walled vascular networks to provide their own blood supply. Rupture of these vessels or invasion of a major vessel by the tumor is the reason bloody sputum occurs.

Is the Amount of Bloody Sputum Important and When Does It Become Life-Threatening?

Our patients understandably feel relieved when they see a small amount of blood and become very alarmed when they see a large amount. Of course, the amount is one of the main factors determining our treatment strategy, but I would like to correct a very critical misconception here: saying that “a small amount of blood is not dangerous” is not medically accurate. Seeing only a thin streak of blood in the sputum (minor hemoptysis) may not put the patient into shock or interrupt breathing at that moment. However, this does not mean that the underlying cause is insignificant. An early-stage lung tumor or an insidiously progressing case of tuberculosis may initially present only as a “stain in the sputum.” Therefore, regardless of the amount, the cause must always be clarified.

When it comes to the urgency of the condition, we encounter the concept of “massive hemoptysis.” Bleeding of more than approximately 600 milliliters within 24 hours or continued bleeding of 100 milliliters per hour, approximately one tea glass, is considered a “red alert” situation for us.

The risks of massive bleeding are as follows:

Asphyxia

Respiratory failure

Hemodynamic shock

Aspiration pneumonia

A very important detail must be emphasized here. People generally think they will die from “blood loss” in such situations. However, in pulmonary bleeding, the primary and fastest fatal danger is not blood loss but suffocation. The total volume of the trachea and main airways is quite limited, approximately 150-200 ml. Therefore, even a glassful of blood that may seem relatively small can completely stop airflow to the lungs if it clots and blocks the main airway. Especially in patients with COPD or individuals whose lung capacity is already reduced due to smoking, even much smaller amounts of bleeding may pose a life-threatening risk. For this reason, as the amount of bleeding increases, the time required to reach the hospital should be minimized.

Why Does Pulmonary Bleeding Not Stop and How Does Vascular Structure Affect This Process?

To understand why pulmonary bleeding can sometimes be very persistent and difficult to stop, it is necessary to examine the remarkable yet equally complex vascular engineering of the lungs. Unlike other organs in our body, the lungs have two separate circulatory systems. This dual structure is both the source of life and a factor that complicates matters in disease.

The first system is the “pulmonary artery” system, which comes from the right side of the heart and enters the lungs to be oxygenated. This system operates under low pressure and provides 99% of the blood supply to the lungs. Gas exchange takes place here. The second system is the “bronchial artery” system. These vessels supply the lung tissue itself, the airways, and the lymph nodes. Since they arise directly from the aorta, the main artery, they are under “systemic pressure.” In other words, whatever value appears when your blood pressure is measured from your arm is also the pressure inside these tiny vessels.

The vast majority of massive and difficult-to-control bleeding cases we encounter in surgical practice originate from this bronchial artery system. In chronic inflammation, tuberculosis, or bronchiectasis, the body enlarges these vessels and increases their number in order to repair the damaged area. However, these newly formed vessels are both highly tense and thin-walled. Since they are under high pressure, once they begin to bleed, the body’s own clotting mechanisms may be insufficient. They may bleed like pressurized water flowing from a tap. Our main goal in treatment is generally to control these high-pressure leaks.

Can a Fungal Ball (Aspergilloma) Cause Bloody Sputum?

This is a special condition that we frequently encounter in clinical practice in Türkiye but that is not widely known among the public. In patients who have previously had tuberculosis and recovered, permanent spaces or cavities may sometimes remain in the lung tissue. These spaces are ventilated but vulnerable areas. Certain fungal spores naturally present in the air, Aspergillus species, may settle in these cavities through inhalation.

Over time, these fungi multiply within the cavity and form a mass or ball; this is called a “mycetoma” or “fungal ball.” This fungal ball can move freely within the cavity where it is located. As it moves, it rubs against the cavity wall and mechanically erodes the blood vessels there. In addition, certain toxins and enzymes secreted by the fungus weaken the vessel walls. As a result, a patient may live for years without any complaints and then suddenly experience very severe and life-threatening bleeding one day. Bleeding caused by aspergilloma is generally recurrent and does not respond to simple medical treatments. It is often one of the most challenging cases in thoracic surgery and frequently requires surgical intervention.

What Will Be Done for Diagnosis When I Visit the Doctor?

When you visit a healthcare institution with a complaint of bloody sputum, your doctor’s priority is to ensure your safety before determining the cause of the bleeding. Are you breathing comfortably, is your blood pressure stable, and is your oxygen level sufficient? These are the first parameters evaluated. Once stabilization is achieved, the process of searching for the source of the bleeding begins, almost like a detective investigation.

The methods used in the diagnostic process are as follows:

Physical examination and medical history

Chest X-ray

Computed Tomography (CT)

Flexible Bronchoscopy

Rigid Bronchoscopy

Blood tests and coagulation tests

The chest X-ray obtained in the first step gives us a general idea, but it is often insufficient to show the pinpoint source of the bleeding. At this point, our “gold standard” imaging method is Computed Tomography (CT). Tomography creates a three-dimensional map of the lungs. It clearly shows which lobe the bleeding is in and whether there is a tumor, infection, bronchiectasis, or vascular anomaly in that area.

Following radiological imaging, the inside of the airways often needs to be visualized directly. This is where “Bronchoscopy” comes into play. The airways are examined by entering through the nose or mouth with a thin, flexible device equipped with a light and camera at its tip. This procedure is used for both diagnostic and therapeutic purposes. We visually confirm the suspicious area seen on tomography and determine with certainty whether the bleeding is coming from the right or left side. Even if there is no active bleeding, we may find the source by following traces of blood. In addition, if a tumor is suspected, a biopsy is taken during this procedure to confirm the diagnosis.

What Are the Treatment Methods and Is Surgery Necessary?

Not every case of bloody sputum is taken to the operating room. In fact, the approach of modern medicine is to begin treatment with the least invasive method possible. The treatment plan is shaped entirely according to the severity and cause of the bleeding and the patient’s general health condition.

The treatment approach for mild bleeding caused by infection is as follows:

Antibiotic treatment

Cough suppressants

Bed rest

Hemostatic medications

If the bleeding is more severe or recurrent, we work in collaboration with Interventional Radiology before surgery. The method called “Bronchial Artery Embolization” (BAE) provides significant comfort for our patients. The procedure is performed by entering through the groin as in angiography, locating the problematic bleeding vessel in the lung, and closing it with special embolic materials. In many patients, this procedure stops the bleeding immediately and provides recovery without the need for surgery, or buys time to stabilize the patient before surgery.

However, in some situations, surgery is unavoidable and offers the most definitive solution. The situations in which thoracic surgery is required are as follows:

Massive and uncontrollable bleeding

Presence of a tumor (Resection)

Fungal ball (Aspergilloma)

Bronchiectasis resistant to medical treatment

Traumatic lung ruptures

The aim of surgical treatment is to preserve as much healthy lung tissue as possible while removing the diseased and bleeding area. Depending on the extent of the disease, only a small wedge-shaped portion (wedge resection), an entire lobe (lobectomy), or, rarely, an entire lung (pneumonectomy) may be removed.

What Should Be Done at Home in an Emergency and What Position Should Be Used?

If you are at home and you or someone nearby suddenly begins coughing up a large amount of blood, panic is your greatest enemy. The first and most important thing to do is call 112 Emergency Services. The position in which the patient is placed until the medical teams arrive is of vital importance.

The points to consider in an emergency are as follows:

Calming the patient

Placing the patient on the bleeding side

Preventing the blood from being swallowed

Keeping the inside of the mouth clean

The most critical point here is the lying position. If you know which lung the bleeding is coming from, for example, if the patient has a known right lung disease, you should place the patient on the bleeding side. In other words, if the right lung is bleeding, the patient should lie on the right side. This may seem illogical to you, and you may think, “the bleeding area should not be compressed.” However, the medical logic is exactly the opposite.

The reason we place the bleeding side downward is to allow the blood to remain trapped in that area under the effect of gravity and to prevent it from filling the healthy lung that remains above. In this way, you ensure that at least one lung stays clear and that the patient can continue breathing. If you place the patient on the healthy side, blood draining from the bleeding lung may fill the healthy side as well, putting the patient at risk of suffocation. If you do not know which side is bleeding, never place the patient flat on the back; sitting the patient slightly leaning forward or turning them onto the side makes it easier for the blood to be expelled.

What Is the Recovery Process Like and What Should Be Considered?

After an episode of bloody sputum has been overcome or the necessary surgical or interventional treatments have been performed, the recovery process should be addressed as a whole. Simply stopping the bleeding is not enough; managing the underlying disease and protecting lung health require lifelong discipline.

First of all, I must address smoking. Smoking is the greatest obstacle during the recovery process. Smoke further damages the already irritated and sensitized airway walls, disrupts the vascular structure, and delays healing. It also multiplies the risk of cancer recurrence or new infections. Quitting smoking is the most effective measure for reducing the risk of recurrent bleeding.

Diet also plays a critical role in tissue healing. In particular, patients with a history of tuberculosis or chronic infection often have reduced body resistance.

The points to consider in nutrition are as follows:

High-protein foods

Foods rich in vitamins C and A

Iron supplements

Plenty of fluid intake

Anemia caused by blood loss may make the patient feel weak. Therefore, replenishing iron stores is important. Respiratory physiotherapy is of great importance in patients who have undergone surgery. Using breathing exercise devices with balls inside, which we call “Triflo,” opens collapsed air sacs and increases lung capacity. Instead of staying confined at home after surgery, taking short and non-strenuous walks helps ventilate the lungs and also reduces the risk of blood clot formation in the legs.

Frequently Asked Questions

Which diseases can bloody sputum (hemoptysis) indicate and when should it be taken seriously?

Bloody sputum (hemoptysis) may be a sign of lung infections, bronchiectasis, tuberculosis, lung cancer, pulmonary embolism, and certain heart diseases.
