This condition is often asymptomatic, although hormonal imbalances and systemic symptoms may develop in some cases. Diagnosis is established through imaging studies and histopathological confirmation.

Symptoms of adrenal metastasis in lung cancer are generally nonspecific and may include fatigue, weight loss, or flank pain. Functional adrenal involvement is rare; however, the risk of adrenal insufficiency increases in cases of bilateral involvement. Clinical evaluation requires a multidisciplinary approach.

Diagnostic methods for adrenal gland metastasis include computed tomography, magnetic resonance imaging, and PET-CT. Differentiating the detected mass from a benign adrenal adenoma is important. When necessary, histological confirmation is achieved through fine-needle aspiration biopsy.

Treatment of adrenal metastasis from lung cancer is planned according to the stage and extent of the disease. Systemic chemotherapy, immunotherapy, and targeted therapies are the primary treatment options. In selected cases with isolated metastasis, surgical adrenalectomy or stereotactic radiotherapy may be performed.

<div id="ez-toc-container" class="ez-toc-v2_0_89 counter-hierarchy class="ez-toc-section" id="Why_Does_Lung_Cancer_Metastasize_to_the_Adrenal_Glands">Why Does Lung Cancer Metastasize to the Adrenal Glands?

The circulatory system in the human body resembles a vast and uninterrupted highway network. The lungs are extraordinarily rich in blood vessels because all of the body’s deoxygenated blood passes through them within seconds to become oxygenated. As lung cancer grows, it easily comes into contact with this rich vascular network, penetrates the walls of the capillaries, and enters the bloodstream. Once cancer cells enter the circulation, they are carried throughout the body by the flow of blood.

Although the adrenal glands are anatomically small, they receive an exceptionally rich blood supply because they continuously produce hormones that regulate stress responses, blood pressure, and metabolic functions. A significant portion of the body’s blood flow constantly passes through these glands. This dense vascular environment acts almost like a filter, providing circulating cancer cells with an ideal place to slow down and attach. Once cancer cells settle within the adrenal tissue, they rapidly multiply using its abundant oxygen and nutrient supply, eventually forming a new tumor, known as a metastasis. This biological and anatomical predisposition is the main reason why the adrenal glands, together with the bones, brain, and liver, are among the organs most commonly affected by lung cancer metastasis.

Are Adrenal Masses Always Metastases from Lung Cancer?

When an adrenal mass or nodule is detected during routine imaging in a person diagnosed with lung cancer, it naturally causes considerable anxiety. However, assuming that every lesion or mass seen on imaging is definitely a cancer metastasis is a major misconception. As people age, various benign and completely harmless masses may develop in different organs.

The vast majority of adrenal masses are benign growths known as adenomas, composed of normal cells that grow slowly and have no ability to spread to other organs. Even healthy individuals with no history of cancer may incidentally have adrenal adenomas. In addition, adrenal cysts, hemorrhages, and less common benign tumors such as pheochromocytomas may also appear as masses on imaging studies. Therefore, finding an enlarged adrenal gland or nodule on a radiology report should not immediately lead to pessimism. The role of modern medicine is to determine whether the detected mass truly represents a malignant cluster of cells originating from the lung or a harmless benign lesion that has remained silently present for years. This distinction is the most critical step because it fundamentally changes the patient’s treatment strategy.

What Are the Symptoms of Lung Cancer Adrenal Gland Metastasis?

During the early stages, when the disease first spreads to the adrenal gland, the metastatic lesion is usually very small and causes no physical symptoms, allowing the condition to progress silently. However, as the tumor enlarges or, more rarely, affects both adrenal glands and disrupts hormone production, a condition known as adrenal insufficiency may develop. Since the adrenal glands produce hormones essential for life, hormone deficiency produces very noticeable symptoms. The main signs that may appear at this stage include:

Unexplained weight loss

Persistent nausea

Severe fatigue

Abdominal pain

Back pain

Constant drowsiness

Mental confusion

Persistent cough

Bloody sputum

Shortness of breath

Chest pain

The first group of symptoms listed above is directly related to impaired adrenal function and hormonal imbalance. In particular, reduced cortisol levels can lead to profound fatigue severe enough to prevent getting out of bed, loss of appetite, and unexplained weight loss. The second group of symptoms results from the primary lung tumor pressing on the airways or the lining of the lungs. The presence of one or more of these symptoms warrants prompt and comprehensive medical evaluation.

How Is Lung Cancer Adrenal Gland Metastasis Diagnosed?

One of the greatest contributions of modern technology to medicine is advanced imaging, which forms the cornerstone of cancer diagnosis and staging. Today, the most commonly used and reliable method for mapping the full extent of lung cancer throughout the body and evaluating the nature of a suspicious adrenal mass is Positron Emission Tomography combined with Computed Tomography (PET-CT).

PET-CT operates on a unique principle. Cancer cells divide and multiply much faster than normal healthy cells, requiring enormous amounts of energy, which they obtain by consuming glucose at a high rate. Before the scan, the patient receives a very small dose of radioactive glucose through a vein. The scanner detects where this glucose is consumed most intensely and displays those areas as bright spots on the images. If the adrenal mass absorbs a large amount of the radioactive glucose and appears highly active, this strongly suggests that the lesion is metabolically active and is most likely composed of cancer cells.

However, interpreting these images requires considerable expertise. PET-CT reports include a value called SUVmax, which numerically reflects how intensely the tissue absorbs glucose. Although a high SUVmax increases the suspicion of cancer, caution is necessary, particularly in countries such as Türkiye where tuberculosis has historically been common. Tuberculosis, sarcoidosis, and various severe inflammatory conditions may also consume glucose intensely and appear bright on PET-CT, producing what are known as false-positive findings. Therefore, imaging results alone should never determine the final diagnosis but must always be interpreted together with the patient’s clinical history, physical examination, and tissue sampling when necessary.

Is a Biopsy Necessary for Lung Cancer Adrenal Gland Metastasis?

No matter how advanced imaging technology becomes, it is impossible to determine the genetic and structural characteristics of a cell with complete certainty simply by observing it externally. This issue becomes particularly important in patients whose lung cancer was successfully treated years earlier and who later develop a newly detected adrenal mass during follow-up. Determining whether this new lesion represents a recurrence of the previous cancer or an entirely new disease is essential because each scenario requires completely different treatments, medications, and surgical strategies. The tissue sampling techniques used to eliminate uncertainty include:

Fine-needle aspiration biopsy

Endoscopic ultrasound-guided biopsy

Core needle biopsy

Fine-needle aspiration biopsy is generally performed by advancing a very thin needle through the skin into the mass under ultrasound or CT guidance to obtain cells using a syringe. Endoscopic ultrasound-guided fine-needle aspiration (EUS-FNA) is a more advanced technique in which a flexible endoscope equipped with ultrasound is passed through the mouth into the stomach, allowing safe access to the adrenal gland through the stomach wall without any external incision. Core needle biopsy is preferred when a larger tissue sample is required. These tiny tissue specimens are examined under a microscope by pathologists using special stains, allowing the true nature of the mass to be determined with certainty.

Can Lung Cancer That Has Spread Only to the Adrenal Gland Be Treated Surgically?

According to the traditional approach in medicine, once lung cancer spread beyond the lung to another organ, it was automatically classified as stage IV disease. Surgery was believed to offer no benefit, and treatment was limited to chemotherapy aimed at symptom control. However, advances in surgical techniques, anesthesia, and the understanding of tumor biology have fundamentally changed this concept worldwide.

Modern oncology recognizes a promising concept known as oligometastatic disease. This term describes cancers that, despite entering the bloodstream, have spread only to a single organ or a very limited number of sites rather than disseminating throughout the body. Lung cancer that has spread exclusively to one adrenal gland without involving any other organs is one of the best examples of this condition. Carefully selected patients with adequate cardiac and pulmonary function and disease limited to the lung and adrenal gland are now considered suitable candidates for surgery. Complete removal of both the primary lung tumor and the adrenal metastasis offers these patients the possibility of long-term survival and even cure.

What Are the Surgical Options for the Lung in the Treatment of Lung Cancer Adrenal Gland Metastasis?

Once oligometastatic disease has been confirmed and surgery has been planned, the first and most important objective is the complete removal of the primary lung tumor with adequate surgical margins. The lungs consist of independent anatomical sections known as lobes, with three lobes on the right and two on the left. Depending on the location and size of the tumor and the patient’s postoperative breathing capacity, the following surgical procedures may be performed:

Lobectomy

Segmentectomy

Wedge resection

Bilobectomy

Pneumonectomy

Lobectomy, involving removal of the entire lobe containing the tumor along with surrounding at-risk tissue, is considered the gold standard worldwide. If pulmonary function tests indicate that removing an entire lobe would excessively compromise lung function, or if the tumor is very small, lung-preserving procedures such as segmentectomy or wedge resection may be performed. Bilobectomy involves removal of two adjacent lobes when both are affected. Pneumonectomy, which removes an entire lung, is reserved for the most challenging situations where the tumor is centrally located and cannot be safely removed by other techniques. Because pneumonectomy reduces breathing capacity by approximately half, it is performed only when absolutely necessary.

How Is Adrenal Gland Metastasis Surgery (Adrenalectomy) Performed?

Once the primary lung tumor has been controlled, the adrenal metastasis is removed during either the same surgical session or a second operation several weeks later. Surgical removal of the adrenal gland is called adrenalectomy. As with lung surgery, advanced technology plays a central role. Depending on the patient’s anatomy, tumor size, and relationship to surrounding tissues, the following techniques may be used:

Laparoscopic surgery

Robotic surgery

Open surgery

Laparoscopic surgery is performed through several tiny incisions in the abdomen or back using high-definition cameras and long surgical instruments. Robotic surgery allows the surgeon to control robotic arms from a console while operating with magnified three-dimensional visualization. The greatest advantages of minimally invasive surgery are reduced postoperative pain and significantly faster recovery. However, if the metastasis is very large or firmly attached to major blood vessels, the kidney, or surrounding organs, open surgery may be required to allow meticulous removal. In such complex cases, converting to open surgery is medically necessary to maximize safety and ensure that no microscopic cancer cells remain.

Why Are Lymph Nodes Important in Surgery for Lung Cancer Adrenal Gland Metastasis?

The key to successful cancer surgery is not only removing the visible tumor but also controlling the potential pathways through which cancer spreads. The lymphatic system carries immune cells that fight infection, while lymph nodes function like security checkpoints along this network. Lung cancer cells often travel through the lymph nodes located in the mediastinum before or alongside their spread through the bloodstream.

Patients are classified according to lymph node involvement during surgical planning. If cancer cells have spread directly from the lung to the adrenal gland without involving mediastinal lymph nodes, this is known as N0 status. Patients with N0 disease derive the greatest benefit from surgery and have the highest chance of long-term survival. However, if cancer cells are found in lymph nodes surrounding the airways (N1 or N2 status), this strongly suggests microscopic spread elsewhere in the body. In such patients, adrenal surgery generally does not improve survival and may unnecessarily reduce quality of life through additional surgical trauma. Therefore, procedures such as mediastinoscopy are often performed before surgery to confirm that the mediastinal lymph nodes are free of cancer.

What Is the Difference Between Synchronous and Metachronous Lung Cancer Adrenal Gland Metastasis?

The timing of metastatic spread is a crucial factor in predicting tumor behavior, disease progression, and long-term outcomes. Based on when metastases appear, they are classified into two categories:

Synchronous metastasis

Metachronous metastasis

Synchronous metastasis refers to adrenal metastasis detected at the same time as the initial diagnosis of lung cancer. This usually indicates aggressive tumor biology and rapid disease progression, making treatment more challenging. Metachronous metastasis, on the other hand, develops months or even years after successful treatment of the original lung cancer. It suggests slower-growing tumor cells and prolonged control by the patient’s immune system. Consequently, patients with metachronous metastases generally have better long-term survival following surgery than those with synchronous disease.

What Are the Success Rates After Surgery for Lung Cancer Adrenal Gland Metastasis?

When faced with stage IV lung cancer, patients and their families understandably seek concrete information about prognosis. Although statistics represent population-based outcomes, they are valuable in demonstrating the potential benefits of treatment. In the past, patients with untreated metastatic disease had very poor five-year survival rates. Today, carefully selected patients undergoing comprehensive surgical treatment have dramatically improved outcomes.

Among appropriately selected patients with isolated adrenal metastasis who undergo complete removal of both the lung tumor and adrenal metastasis, five-year survival rates may reach approximately 30% to 50%. Outcomes are especially favorable in patients with N0 status, meaning there is no cancer involvement in mediastinal lymph nodes. Another positive prognostic factor is normalization of tumor markers such as CEA or CA 19-9 after treatment. These findings demonstrate that, even in metastatic disease, complete recovery is possible with proper patient selection and optimal surgical management.

What Drug and Radiation Therapy Options Are Available for the Treatment of Lung Cancer Adrenal Gland Metastasis?

Surgery alone is not sufficient in the treatment of lung cancer. Management always requires collaboration among thoracic surgeons, medical oncologists, and radiation oncologists. While surgery removes visible tumors, additional treatments are necessary to eliminate microscopic cancer cells that may remain hidden within tissues or the bloodstream. The primary medical treatment options include:

Targeted therapies

Immunotherapy

Chemotherapy

Radiotherapy

Targeted therapies are modern medications used when genetic testing identifies specific mutations within the tumor, allowing selective destruction of cancer cells while minimizing damage to healthy tissues. Immunotherapy is a revolutionary treatment that activates the patient’s own immune system to recognize and attack cancer cells. Conventional chemotherapy may also be administered before surgery to shrink tumors or after surgery to reduce the risk of recurrence. Radiotherapy uses high-energy radiation to prevent local recurrence or relieve symptoms caused by metastatic lesions in distant organs.

What Is the Recovery Process Like After Surgery for Lung Cancer Adrenal Gland Metastasis?

Following successful surgery, patients enter a recovery and follow-up period that requires patience, attention, and discipline. Depending on whether the procedure was performed using minimally invasive or open techniques, patients are usually ready for discharge within about one week. However, complete tissue healing, resolution of incision-related discomfort, and return to normal physical strength generally require one to two months. The main elements of recovery include:

Regular use of prescribed pain medication

Respiratory physiotherapy

Breathing exercises

Short walks

Regular outpatient follow-up visits

Computed tomography scans

Blood tests

Because postoperative pain may discourage patients from taking deep breaths, consistent breathing exercises using devices such as an incentive spirometer are essential for restoring lung capacity and preventing infections. During the first years after surgery, regular thoracic surgery and medical oncology follow-up appointments are typically scheduled every six months to ensure that the disease has not silently recurred elsewhere in the body.
